At the title screen, press Up, Right, Down, Left, I, II to access a hidden options menu that contains a level select toggle (third option) and sound test (last two options), among other things.

The options translate to the following: 

Difficulty	Thorny Path
Lives		6
Choose Region	Yes
Music		00
Voice		00

This translates to "48 Ueno".

If enabled in the hidden options menu described above, the level select will appear when starting the game.
